UMWC Photo Policy and General Waiver and Release Form

Photo/Video policy:

University of Minnesota Women’s Club (“UMWC”) 

UMWC reserves the right to use any photograph or video taken at any organized or sponsored event on its website and/or in club publications or other communications produced or used by UMWC including but not limited to: brochures, newsletters, posters or other signage, media presentations and promotions. In addition, UMWC reserves the right to identify, by name, the person(s) included in any such photograph or video. OPTING OUT: Going forward a person attending a UMWC event who does not wish to have their image and/or their name recorded and used in publications or media productions must make their wishes known to the photographer. An individual may opt out of having their image in all future publications or media productions by contacting the Newsletter Editor in writing of their intentions. Individuals may not opt out with respect to publications or media productions previously or currently in existence. By participating in a UMWC event and failing to make your desire to opt out, in the manner described above, you are agreeing to release, defend, hold harmless and indemnify UMWC from any and all claims involving the use of your image or name.

Waiver and Release of Liability

University of Minnesota Women’s Club (“UMWC”) 

As a member of UMWC, I have available to me a wide variety of activities, events and programs, that may take place indoors or outside at various venues (commercial and private), and may involve food, transportation and/or lodging supplied by and/or arranged by UMWC or members of UMWC with outside vendors (collectively the “Programs”). I acknowledge, understand and accept that my participation in any aspect of the Programs, may expose me to dangers, hazards, and risks including, but not limited to, the risks of illness, physical injury, temporary or permanent disability, death, and economic and property loss.  I understand and agree that I am personally responsible for my own actions and well-being.

In consideration for being allowed to participate in the Programs, I, for myself and on behalf of my respective heirs, successors, assigns, and personal representatives, hereby forever release, acquit, discharge, covenant not to sue, and agree to indemnify and hold harmless for any and all purposes UMWC and its directors, officers, agents, and volunteers in official and individual capacities (“Releasees”) from any and all liability whatsoever for any and all damages, losses, or injuries (including but not limited to death) to persons or property or both, including but not limited to any and all claims, demands, actions, causes of action, damages, losses, injuries, costs, expenses, and attorney’s fees, that result from, arise out of, or are related to my participation in the Programs, my travel to or from the Programs, or my presence on premises owned,  supplied, operated or arranged by Releasees, INCLUDING BUT NOT LIMITED TO DAMAGES, LOSSES, OR INJURIES SUSTAINED AS A RESULT OF THE ORDINARY NEGLIGENCE OF RELEASEES.
